What Is Diabetic Peripheral Neuropathy?

Diabetic peripheral neuropathy is nerve damage caused by high blood sugar levels over time. It’s one of the most common complications of diabetes and usually affects the nerves in the feet and legs first, and sometimes the hands.

When blood sugar stays elevated, it damages the small blood vessels that supply the nerves. As a result, the nerves can’t get enough oxygen and nutrients, which leads to:

  • Numbness, tingling, or burning sensations in the feet or hands
  • Sharp or shooting pain that may worsen at night
  • Muscle weakness or loss of balance
  • Cuts or sores on the feet that don’t heal well (because of reduced sensation)

Signs You Might Have Peripheral Neuropathy

Sensory Symptoms (most common)

  • Numbness in the feet, toes, or hands
  • Tingling or “pins and needles” sensations
  • Burning pain (often worse at night)
  • Sharp, shooting, or electric-like pains
  • Increased sensitivity to touch, so even light touches may hurt
  • Reduced ability to feel temperature changes

Motor (movement) Symptoms

  • Weakness in the legs or feet
  • Difficulty walking, climbing stairs, or standing for long periods
  • Muscle cramping or twitching
  • Frequent tripping or stumbling
  • Loss of balance or coordination

Autonomic Symptoms (less common, but possible)

  • Changes in sweating (too much or too little)
  • Digestive issues like bloating, constipation, or slow digestion
  • Dizziness when standing up (due to blood pressure changes)
  • Heat intolerance
